Responsibilities : As a Client Service Coordinator, your role will be pivotal in connecting patients, families, and healthcare professionals. You will: - Engage professionally with patients, families, staff, and funding organizations. - Efficiently process funder referrals and updates in our EMR system (Procura). - Collaborate with Supervisors and field staff to evaluate and assess service needs. - Schedule care providers based on client requirements, geographic location, staff skills, and availability. - Ensure consistent caregiver assignments for seamless service delivery. - Keep patients and staff informed about both initial and ongoing schedules. - Optimize staff schedules to align with client care needs. - Maintain accurate documentation within the EMR system. - Address and resolve patient concerns promptly and follow proper escalation procedures.

Requirements : We seek individuals who are passionate about enhancing patient care with the following qualifications: - Exceptional customer service skills. - A proactive approach to learning and adapting. - Strong scheduling capabilities. - Availability for rotating shifts between Monday and Sunday, from 5 AM to 10 PM. - Hybrid work model: 2-3 days a week at our Markham office (340 Ferrier Street) after a successful three-month probation period.
